CEO, Licensed Assisted Living Director, Co-Owner

Josh Cesaro-Moxley

Josh Cesaro-Moxley holds a Bachelor of Business Administration from the University of Minnesota Duluth, and is a Licensed Assisted Living Director, Licensed Real Estate Agent, Licensed Food Manager, and Certified Nursing Assistant. With a deep passion for supporting residents and their families, Josh ensures a welcoming, caring environment at Heart to Home. He plays a key role in management support, business development, and community outreach, while also serving as the business liaison. Beyond his leadership at Heart to Home, Josh actively contributes to various boards, including the Saint Paul Senior Workers Association (SPSWA), and is currently the President of the Residential Care Providers Network of Minnesota (RCPN). He also supports fundraising efforts for organizations such as the Alzheimer’s Association, American Cancer Society, and Alzheimer’s Research Center. Outside of his professional endeavors, Josh enjoys quality time with his wife and five children.

Nick Cesaro-Moxley

Nick Cesaro-Moxley holds a Bachelor of Arts degree, a contractor’s license, and is a retired nationally registered paramedic. With over 35 years of experience as a residential builder, Nick brings a wealth of expertise in building design and construction to Heart to Home. As the visionary behind our custom-designed senior care homes, his exceptional craftsmanship has earned him recognition in the Parade of Homes and within the local community. Nick’s focus is on ensuring our residents have safe, beautiful, and comfortable living spaces. In addition to overseeing the construction and maintenance of our homes, he is a proud co-owner alongside his son, Josh. Outside of work, Nick enjoys boating, traveling to Maryland, and cherishing time with his thirteen grandchildren.

Clinical Nurse Supervisor South Campus

Misty Burnette, RN

Misty Burnette is a highly skilled Registered Nurse (RN) licensed in the state of Minnesota, with over a decade of dedicated service to Heart to Home since 2011. In her role, Misty provides invaluable ongoing support and training for new staff while overseeing nursing services across Heart to Home's four locations. Her extensive experience includes leadership positions with local hospice agencies and larger memory care facilities, where she honed her expertise in elder care. Known for her unwavering work ethic and deep compassion, Misty is deeply committed to enhancing the lives of older adults. Outside of her professional life, she enjoys gardening, reading, spending time with her family, and traveling.

Operations Manager & Dementia Care Specialist

Angie Burnette

Angie Burnette, LALD, CAEd, CAC, CNA, CFM, has been an essential member of the Heart to Home team since 2010. She has held various roles, including Certified Nursing Assistant and House Manager, and currently serves as our Operations Manager and Licensed Assisted Living Director. Angie is a Certified Alzheimer’s Caregiver and Dementia Care Trainer, accredited by the National Certification Board for Alzheimer’s Care.

Her expertise extends beyond her certifications as a Licensed Food Manager and Certified Nursing Assistant, demonstrating a natural gift for providing compassionate care to our residents. With years of experience across different long-term care settings, Angie discovered her true passion for delivering personalized, one-on-one care at Heart to Home.

She has completed extensive training in dementia and hospice care, both in-house and through respected organizations like Care Providers of Minnesota and LeadingAge Minnesota. Angie is an exceptional trainer and mentor to our staff, working diligently to ensure that staffing and training remain top priorities. Outside of her professional responsibilities, she enjoys traveling and spending quality time with her family.

Minellie

Minellie Ayala-Arroyo has been an integral member of the Heart to Home team since 2012. With experience as a Certified Nursing Assistant and House Manager, Minellie brings a wealth of knowledge from various long-term care settings. However, she found her true passion in managing the day-to-day operations at Heart to Home, where she strives to create a nurturing environment for both residents and staff. She has received extensive training in dementia and hospice care through both in-house programs and organizations like Care Providers of Minnesota and LeadingAge Minnesota. Known for her hard work, compassion, and positive attitude, Minellie is dedicated to ensuring that your loved one receives the highest quality care at Heart to Home.

Staffing & Activities Coordinator

Susan Heutmaker

Susan has over 10 years of experience in the senior care industry, having worked as a Certified Nursing Assistant (CNA), House Manager, and Licensed Assisted Living Director (LALD). Her extensive background in care, combined with her specialized training in activities, allows her to create engaging and fun programs for our residents. Along with her activity coordination, Susan also manages the staff schedule to ensure smooth operations.

What she loves most about working at Heart to Home is the opportunity to truly connect with each resident on a personal level. In our smaller, intimate setting, she’s able to build meaningful relationships with residents as friends rather than just patients.

Susan, originally from Northfield, Minnesota, now lives in Inver Grove Heights with her husband Robert and their cat, Luna. Outside of work, she enjoys yoga and staying active with various outdoor activities.

House Manager, Mendakota Manor

Rane

Rane has over six years of experience in dementia care and brings a wealth of knowledge to Heart to Home. Before joining us in 2020, she held a management role at a group home for adults with dementia. Her deep connection to Heart to Home and its residents made transitioning into the role of House Manager a natural fit. Rane is passionate about fostering a warm, home-like atmosphere, where meaningful relationships and strong bonds are built with each resident. Known for her caring and kind-hearted nature, Rane’s infectious laughter fills the halls as she always strives to brighten the day of those around her. She takes great pride in getting to know her residents and creating lasting connections with them. Eager to continue growing and learning, Rane looks forward to all the experiences and opportunities that lie ahead at Heart to Home. In her free time, she enjoys spending quality time with her family.

Ashley

Ashley has been a dedicated team member at Heart to Home since 2015, working across all four of our locations. Her extensive experience has given her a profound understanding of what makes Heart to Home truly special—the strong, personal relationships between our staff and residents. Known for her meticulous attention to detail, Ashley’s positive attitude and infectious laugh bring energy and warmth wherever she goes. Both our residents and colleagues deeply appreciate her presence and enthusiasm. Outside of work, Ashley loves spending quality time with her friends and family!

Abbey

Abbey has been a dedicated nursing assistant at Heart to Home since 2017, bringing with her a wealth of experience from various long-term care settings, including memory care and assisted living facilities. What she loves most about working at Heart to Home is the consistency of her assignments, where she has the opportunity to work with the same small group of residents each day. This allows her to build deep, meaningful relationships and to better anticipate and respond to the evolving needs of each resident. Abbey takes great pride in the high-quality senior and memory care provided at Heart to Home. Outside of work, she enjoys spending time with her family, embracing the outdoors, and indulging in her passion for hunting and fishing.

Lisa Jones

Lisa has been a Licensed Practical Nurse (LPN) since 2017, with experience in a variety of long-term care and group home settings. At Heart to Home, she plays a key role in delivering personalized nursing care to our residents and assisting our Clinical Nurse Supervisor. Lisa collaborates closely with both the direct care staff and resident families to ensure each resident receives the highest standard of care. Her dedication and compassion make her a valued member of the team. In her free time, Lisa enjoys spending quality moments with her friends and family.

Evening Nurse Supervisor

Maura McDevitt

Maura has been an integral part of the Heart to Home team since 2016. She began her journey with us as a Certified Nursing Assistant (CNA) while pursuing her nursing degree. After earning her LPN, Maura was promoted to Evening Nurse Supervisor, where she continues to shine. Her compassionate approach and natural ability to mentor and guide new staff are invaluable assets to our team. Maura's attention to detail ensures that our residents consistently receive the highest standard of care. We are proud to have her as part of our team, providing exceptional care with a personal touch.

Priscilla Amankwah-Akuffo, RN

Priscilla brings several years of valuable experience to Heart to Home, having worked in both our facility and a Transitional Care Unit. Her diverse background has allowed her to sharpen her nursing expertise, particularly in caring for older adults. She began her nursing journey at Inver Hills Community College, earning her ADN, before completing her BSN at Metro State University. Priscilla thrives in the close-knit, home-like environment at Heart to Home, where she enjoys building meaningful connections with each resident. Above all, her greatest joy comes from brightening their day and putting a smile on their faces!

Robert Heutmaker

What I truly enjoy about working at Heart to Home is the opportunity to connect with and get to know the residents on a personal level. It brings me great pride knowing that I can make a positive impact on their lives, and I find every day to be unique and rewarding. Outside of work, I love spending quality time with my wife, Susan. My hobbies include sports, camping, golfing, music, singing, and anything that allows me to enjoy the great outdoors.
